Using Ultra-widefield Retinal Imaging (optomap®) in Cataract Patients

What is a Cataract

A cataract is a clouding of the eye’s natural lens, which is normally clear. Cataracts remain the leading cause of vision loss and the US Centers for Disease Control and Prevention (CDC) estimates that 20.5 million Americans over the age of 40 suffer from cataract in either one or both eyes.

Pre-operative Assessment

With any surgical procedure, a full diagnostic exam is standard. Cataract surgery is no different. The retina – situated posteriorly in the eye - can be challenging to evaluate with a dense cataract, precluding important clinical information such as diabetic retinopathy, holes, tears, or other valuable information which may affect surgical outcomes. Preoperative care is important to cataract surgery outcomes – making it necessary to look for retinal pathology that may affect vision, influence cataract treatment decisions or require co-management with a retinal specialist. Retinal imaging can facilitate a more comprehensive exam as well as document findings. optomap has been shown to have various benefits over traditional white light imaging in patients with cataracts1,4.

Post-operative Monitoring

From assessment of IOL positioning to documentation optomap can provide an efficient, patient-friendly solution.  Even in uneventful surgery, soluble inflammatory factors are released which may compromise the blood retinal barrier (BRB) integrity potentially causing vascular leakage. If present, secondary retinal swelling and vitreous opacity can be assessed with optomap fa (fluorescein angiography) to ease the common postoperative complaints of floaters and blurred vision and educate cataract and refractive lens exchange patients.4

Additionally, 53% of eyes in asymptomatic patients post cataract surgery have peripheral lesions (non-surgical related retinal breaks and/or detachments) that can be visualized on optomap, supporting the preservation of improved visualized acuity.5
